There is currently an issue with "A Simple Re-Quest". The achievement has a bug that will sometimes cause it to reset if you enter a Cross-Realm Battleground. We have a fix in place for this that will go live with patch 3.0.3 (soon). In the meantime, if you absolutely want this achievement, the best thing to do is to not enter a BG while working on it.

One thing I've noticed--but have been unable to prove due to my schedule--is that the achievement appears to be on a 24-hour hard timer, and not on a calendar timer.

For example, if I log in at do quest X at 4:00 pm on Monday it will say 1/5. If on Tuesday, I log in at 3:30 PM it will still say 1/5, but if I log in at 4:30 pm it will say 0/5.

Might be observer error, or small sample size, but i've had it reset on my at least 5 times already, and I do at least 1 daily every calendar day.

I've only been in 2-3 BGs since the patch, and they were all on the same day.

I think you're right about that. If I do my dailies at the same time every day, I get credit for the achievement. If I'm off by a few hours, it resets. I've been up to 4/5 on two different occasions and on the next day when I started my dailies I was back to 0/5.

I have not entered any BGs since 3.0.2 dropped.

This achievement is not based on doing the same quest every day for 5 days, but rather based on doing any daily for 5 consecutive days.
However as other people have noted, you have to do a daily quest within 24 (real) hours of the previous daily quest.
The easiest way to do this is to do a daily in the morning, and one in the evening for 5 days.

Avoid BG's too just to be safe, although personally I did not notice them causing a problem.

Is the 24 hour timer going to be changed with the fix too? Not a big deal, I could just do a daily late on the first day and do one earlier and earlier as the days go on.

I was wondering what the heck was going on. I thought maybe I had gone crazy.

What about those who have never entered a BG, its resetting for us too.

I had finished the acheivement got the adknowledgement and it was still reset. I do not have the ach. anymore. I have never been in a battleground but I have been in a few instances. so, maybe going into instances resets it too.

But, I still want to know why it erased the fact that I actually got the ach.
