When in Unholy Presence, if two Blood Strikes are made, the talent Blood of the North will only convert the first blood rune to a death rune. The second blood strike will not.
To reproduce, you have to do them one right after the other. It's almost like BotN's effect has a cooldown just slightly longer than the GCD when in Unholy Presence, so if you're spamming the key, it won't give you the proper number of death runes.

I am having the same issues with Blood of the North not functioning properly. It always happens to the second Blood Rune in the rotation and it does really mess with the rotation. Blood Pres.

Having the same issue.
Only way i have been able to get around this as an interim (still equates to a DPS loss though) is to BS then either Oblit or RP dump then BS again. Doing this I have never had the second Blood Rune not convert.
Would also be interesting to see how ppls latency is affecting this. I run at about 350 ms (oceanic) and have found that while the second BS in BP can still regenerate as a Blood Rune that it is a lot less likely (anecdotally about 1/5 times)

I'm having the same problem.
it's caused me to drop from about 3400ish dps to about 2k. that's very very bad.
mage : i can sheep the mob
rogue : i can sap the mob
hunter : i can trap the mob
warlock : i can use my succubus to seduce the mob
warrior : I'll kill the damn mob, what more do you want?

The pestilence hotfix was nothing more than napkin fix that was poorly thought out. It looks like Blizz was dumb enough to simply add an internal cd for blood rune to death rune conversion (at least for BotN) and it happens to be 1.5s. This causes the bug to occur almost all the time in UP and sometimes in BP due to latency etc. It greatly impacts our game play so please implement a fix. This thread at least deserves a blue response.

I googled this post up after testing out damage output on special hit attacks.
I had problems converting both blood runes into death runes in succession.
If I waited a fraction of a second (probably more like half a second) after the 1 second gcd in unholy presence was up from casting the first blood strike, I could cast blood strike and convert the second rune.
If I spam blood strike, the first one would convert into a death rune, the second one would be spammed until it went off, then the second blood rune stays as a blood rune and won't convert, this is not a graphical issue either, I tried casting icy touch with this rune after putting all other runes on cd, but sadly it doesn't work since it truly is still a blood rune.
This all works fine for me in blood and frost presence. It converts the first and second blood runes into a death rune if spamming blood strike as fast as I could hit the binding key.
Just unholy presence, there seems to be an internal cooldown on it, like others say, that disables it from converting into a death rune.
Blood of the North
Increases Blood Strike and Frost Strike damage by 10%. In addition, whenever you hit with Blood Strike or Pestilence there is a 100% chance that the Blood Rune will become a Death Rune when it activates. Death Runes count as a Blood, Frost or Unholy Rune.
This doesn't seem like a 100% chance to me. Please hotfix this bug.
Just like the way you hotfixed my Ret Paladin's double Seal of Righteousness Proc on a Judgement cast into only a single proc so quickly.
Or will this be like one of those times when my Shadow Priest had to deal with that broken Mind Flay bug back around BC to WOTLK transition for like 2-3 patches? the whole cast was shifted like half a second to 1 second, so the spell was basically a 3.5-4.0 second cast in order to get the full 3 ticks off. It was awesome. Not.
